How to Create a QR Code for Your Google Form

1

Get your Google Form URL

Open your Google Form in edit mode. Click the "Send" button (top right). Click the link icon (🔗). Copy the URL. Optionally check "Shorten URL" — though a dynamic QR code in QRTRAC provides a cleaner short URL automatically.

2

(Optional) Add UTM tracking parameters

For analytics attribution, append: ?utm_source=qr&utm_medium=print&utm_campaign=event_checkin to your form URL. This lets Google Analytics (and QRTRAC) track where responses came from.

3

Create a dynamic URL QR code in QRTRAC

Log into QRTRAC → New QR Code → Website URL type. Paste your Google Form URL (with UTM if added). Choose dynamic mode so you can update the destination later without reprinting.

4

Customize with your brand

Add your logo, adjust colors, add a CTA label ("Scan to check in", "Scan to give feedback"). Export at 300 DPI PNG or SVG.

5

Place and track

Print on badges, table cards, signage, or product packaging. QRTRAC's dashboard shows every scan — count, device, time, location. Google Forms shows every submission. Together, you see your scan-to-submission conversion rate.

Use Cases: Google Forms + QR Codes

Any form that benefits from instant mobile access at a physical location.

🎟️

Event Check-In

QR on registration confirmation email or printed badge. Scan = instant attendance logging in Google Sheets via Forms.

CTA: "Scan to check in"
📝

Post-Event Feedback

QR on the exit lanyard, receipt, or seat card. Attendees scan and submit feedback before they leave the venue.

CTA: "Scan to rate your experience"
🏥

Patient / Client Intake

QR in the waiting room. New patients or clients scan and complete intake forms on their phone before they're called in.

CTA: "Scan to complete intake form"
🛒

Pop-Up / Market Orders

Farmers markets, pop-ups, craft fairs: QR at your booth links to a Google Form order form. Collect pre-orders digitally.

CTA: "Scan to place your order"

Customer Satisfaction (CSAT)

QR on the receipt or table tent. 30-second satisfaction survey. Scan data shows when customers are giving feedback vs just passing the QR.

CTA: "Scan to rate your visit"
📧

Email List Signup

QR on packaging, print ads, or storefront. Links to a Google Form collecting name + email for your mailing list.

CTA: "Scan to join our list"
🔧

Support / Warranty Registration

QR on product inserts links to warranty registration or support request form. Reduces friction vs typing a URL.

CTA: "Scan to register your product"
🏫

School & Classroom Forms

Permission slips, parent contact forms, assignment submissions — QR codes on handouts make Google Forms actually get completed.

CTA: "Scan to submit"
💼

Job Application Forms

QR in job posting flyers or "Now Hiring" signage links directly to your Google Form application. Low-friction mobile apply.

CTA: "Scan to apply now"

Pro Tip: Pre-Fill Hidden Fields by QR Code

Google Forms allows pre-filled URL parameters. Create a unique QR code for each location, event, or campaign — and have that QR code automatically pre-fill a hidden "Source" field in your form.

Example: You have a satisfaction survey form. You place QR codes at 5 different restaurant locations. Each QR code pre-fills "Location = [Branch Name]" in a hidden field. Now every submission automatically includes which branch the customer was at — no extra question needed.

How to get a pre-filled URL in Google Forms:

In the form editor → three-dot menu → "Get pre-filled link" → fill in the field values → copy link. Use that pre-filled URL as the QR code destination in QRTRAC.

Google Form QR Codes: Frequently Asked Questions

Everything you need to know about creating QR codes for Google Forms.

Q How do I create a QR code for a Google Form?

1. Open your Google Form. 2. Click 'Send' (top right) → link icon → copy the form URL (optionally shorten it with the checkbox). 3. In QRTRAC, create a new Website/URL QR code and paste the Google Form URL. 4. Customize with your logo and colors. 5. Download and place. The QR code opens the form directly in the scanner's mobile browser — no app needed.

Q Can I track how many people scanned my Google Form QR code?

Yes — with two layers of tracking. QRTRAC provides native scan analytics (scan count, device, OS, time, location) regardless of form submissions. To track the full funnel (scanned → submitted), add UTM parameters to your form URL before generating the QR code. In Google Analytics, you'll see sessions from your QR UTM source alongside Google Forms native response tracking.

Q Does the Google Form QR code work on all smartphones?

Yes. The QR code opens the form URL in the user's mobile browser (Chrome, Safari, etc.) — Google Forms is fully mobile-responsive and works on any modern smartphone without requiring the Google Forms app.

Q Can I update the Google Form URL after creating the QR code?

Yes — if you use a dynamic QR code from QRTRAC. Dynamic codes point to a QRTRAC short redirect URL, not the form URL directly. You can update the destination URL (to a new form version, a different form, or any other URL) without changing or reprinting the QR code. Static QR codes encode the form URL directly and cannot be updated.

Q What types of Google Forms work best with QR codes?

The highest-value Google Form + QR combinations: event check-in forms (scan = instant attendee registration), post-event feedback (QR on the receipt or lanyard), in-store feedback (QR at the counter), intake forms (clinics, studios — scan on arrival), order forms (farmers markets, pop-ups), and newsletter signups (QR on business cards or product packaging).

Q Can I pre-fill Google Form fields from a QR code?

Yes — Google Forms supports pre-filled URL parameters. You can create a form URL that pre-populates hidden fields (like source location, product ID, or event name) using Google Forms' pre-fill feature. Generate the pre-filled URL, then create a QR code pointing to it. This is a powerful technique: different QR codes at different locations pre-fill a 'Where did you scan this?' field automatically.
